NO BOOKING IS CONFIRMED UNTIL WE RECEIVE THE TRANSPORT ORDER FORM BELOW AND THE DEFRA PET TRAVEL DECLARATION. THE ORDER FORM MUST BE COMPLETED IN FULL AND SUBMITTED WITH COPIES OF THE PASSPORT PAGES REQUESTED AND A PHOTO OF THE ANIMAL.

The DEFRA PET TRAVEL DECLARATION also needs to be completed and returned via email to [email protected] with your name and order number as subject. 

Every customer if using a passport needs to add their name at the top, chip number top left of the boxed area, place and date is the collection place and date and then sign at the bottom as it appears on the owner page of the passport
